To provide notary services with DNique Notary Service you will need to complete and upload the forms outlined below;
- Current W-9
- Copy of Notary Commission (Stamp will not be accepted)
- Copy of Driver’s License (Must be a color copy)
- Copy of Notary Bond (If required in your state Bond Required States)
- Copy of Errors & Omissions Policy (minimum $25,000 coverage)
- Copy of Background Check
Please note, all documents must be uploaded for your registration to be completed. Incomplete submissions may result in cancellation of your registration.
